Office Appointments & Surgery
Make an Office
Appointment
Call us at (818) 841-3936 on Monday through Friday, between
8:00 AM – 5:00 PM to make an appointment. We accept calls
during the lunch hour. We make every effort to schedule your
appointment as quickly as possible.
New Patient
Before arriving at your first appointment, please review
this list and bring all pertinent information.
• Your
insurance card, driver’s license (or valid picture ID) and
co-payment required by your insurance.
•
A complete and accurate list of your current medications.
•
Contact information for your referring doctor so our
physicians may send office notes and a letter after your
visit.
•
Any tests (x-rays, CT scans, MRIs) relevant to your problem.
•
Completed New Patient Forms.
If you are unable to complete the forms, please arrive 20
minutes before your appointment to give you the time to
complete the forms.
Cancel an
Appointment
If you need to cancel your appointment, please call us at
least 24-hours prior to your appointment.
Coordinate a
Surgery
Once you and one of our Orthopaedic doctors have confirmed a
surgery is required, please ask to speak with Donna
to coordinate the surgery. |
