Insurance & Fees
Thank you for choosing our practice. We are committed
to the success of your medical treatment and care.
Please remember that payment of your bill is part of your
treatment and care with us.
Our physicians are contracted with many Preferred Provider
Organization (PPO) insurance plans.
As such, our office will file insurance claims for major
procedures. We accept patients who are “In Network” and
“Out of Network”. We welcome Medicare, Worker Compensation
plans, and patients who will pay by cash.
Payment Policies
Please read our payment policies before and/or at the time
of your visit.
We accept the following methods of payment: cash, check,
VISA and MasterCard.
We urge you to call your insurance plan so you understand
services that are included and are not included by your
health plan.
If you have questions about payment for services, insurance,
and billing, please call our office and ask to speak with a
financial counselor / insurance department.
